When i write first post about "Create a facebook application".Many friends comment and ask more questions So i think this post will answers to your all quations.
1.If you haven't a Hosting account(free or Pentium )no matter i will learn you how to get free hosting account and create your account professionally.
2.First you want a hosting account .So search on google OR click here to do it.(i learn you how to create app with a free hosting account).
3.Select a Hosting service on google search list.(i selected www.000webhost.com)
4.Create a Index file (index.html) and upload it to your hosting account.
5.Then click here and go to part 1 for next step.
